To be completed by parent/guardian.

To be completed by physician, or you can attach a report provided by your child’s physician.

This form provides helpful information to your child’s counselor.

Release Authorization
Please list others who are authorized to pick up your child.

Shuttle Bus Request
Due by June 1st – this service is available to families who have campers at both CHS and PMC. 

Authorization to Administer Medication Form
Due prior to or on your first day of camp - if your child requires medication during camp, please read, complete and submit this form. You should bring this form directly to the camp nurse, along with the medication in the original container.
