Shuttle Bus Request
Due by June 1st – This service is available to families who have campers attending both CHS and PMC during the same session. Eligible families will receive more information after registering. Please note space is limited.
Authorization to Administer Medication Form
Due on or before your first day of camp. If your child requires medication during camp, complete and submit this form. You should bring this form directly to the camp nurse, along with the medication in the original container.
